Remote Collaboration and Evidence-Based Care

Hello everyone, and welcome to my presentation. I will discuss how to carry out remote collaboration and evidence-based practice in healthcare. This recording is mainly influenced by health disparities between people living in rural areas and those in urban regions. People in rural and remote areas often have limited access to quality healthcare. As such, telehealth services emerge as a proper way to bridge this gap in access to quality care. According to Groom et al. (2021), telehealth provides affordable ways for patients to receive healthcare information and advice from a professional provider. The experience of remote areas lacking sufficient and convenient medical care services and sources is depicted in the case study for this evaluation, which involves a patient with congestive heart failure (CHF). I will explain how the patient can have an effective evidence-based care plan to manage his condition despite being in a remote area.

First, let us review the patient’s issue.

The case study focuses on a male 65-year-old diagnosed with congestive heart failure (CHF). According to Banerjee et al. (2021), CHF is a severe condition that affects the heart, limiting its ability to pump blood as efficiently as it should. The patient lives in a remote area, giving him less access to essential cardiac care. Moreover, he has a history of hypertension. The patient has been experiencing various symptoms, including leg swelling, fatigue, and shortness of breath. Primarily, my focus is ensuring that the patient gets all the resources he needs despite staying in regions associated with significant health disparities.

Next, I want to propose an evidence-based care plan to help manage the identified patient in the case study.

Usually, evidence-based care plans offer nurses vital clinical information about best clinical practices with enhanced possibility of enhancing patient outcomes. The evidence-based care plan should improve patient safety and outcomes regarding the chronic heart failure issue.
